Uterine Fibroid (SALA-E-RAHAM)

UTERINE FIBROIDS  (leiomyomas or myomas ) are the most common pelvic tumors and the most
Common benign tumors in women.

ETIOLOGY
The etiology is largely unknown, but they are estrogen-and progesterone- dependent tumors, very rare before the monarch, common in reproductive life, and frequently regress in size after menopause.

SIGN AND SYMPTOMS
* Symptoms of fibroids are heavy menstrual bleeding, non-cyclic pelvic pain, dysmenorrhea, dyspareunia, urinary symptoms, infertility, anemia, and constipation.
* Fibroid size does not necessarily determine the severity of clinical symptoms.

RISK FACTORS
• Nulliparity
• Early menarche
• Use of Oral contraceptives before 16 years of age.
• Additional environmental factors, such as diet, particularly vitamin D deficiency, and environmental toxins.

PREVALENCE
• It affect over 25% of females in the reproductive age and cause significant morbidity in that population.
• It is estimated that as many as 50% of leiomyomas are symptomatic and represent the most common indication for hysterectomy in many countries.

DIAGNOSIS
 Transabdominal or transvaginal ultrasound is used commonly as it is widely available, inexpensive and usually definitive in diagnosis.
 MRI may be used to delineate the number, size and location of fibroids in certain cases, and hysteroscopy may be useful to distinguish between sub- endometrial fibroids and large endometrial polyps.

Non – SURGICAL TREATMENT
 Symptomatic treatment of heavy menstrual bleeding , with use of non-steroidal anti-inflammatory drugs ( NSAIDs)
 Antifibrinolytic agents including tranexamic acid, or contraceptives steroids including the levonorgestral intrauterine system ( Mirena )
 GnRH agonist
 Selective progesterone receptor modulators

Natural Treatment and Unani Concept
 Sala’in Unani system of medicine and usually belong to Warm Balghami and sodawi ( phelegmatic Swelling ).
 Accumulation of Abnormal Balgham ( Phlegm ) and Sowda in the Rahim ( uterus ) leads to Sala’al Rahim , hence it is a Balghami and Sodawi ( Phelegmatic، , and Atrrabilious /cold and dry ) benign swelling .

Natural Treatment
The treatment for Sala ‘ al-Rahim ( Uterine fibroid ) is removal of morbid matter with Joshanda e Mundij and Mushil Balgham ( منضج اور مسہل بلغم و سود).
Hence, inutility patient should be administrated Mundij for 15 to 22 days followed by Mushil and Tabreed alternately. But My experience is something different after Mundhij, I treat patient with blood purifying herbs like Chiretta Shahtra(Androgryafis Paniculeta ) in the form of Joshanda after this, we can give Mushil .
And end with Tabreed.
In my opinion fibroid is an atrabilious disease means sowdawi disease abnormal sowada deposits in the uterus.

Causes:
Now a days pregnant women are taking lots of Iron and Calcium supplements during pregnancy and they couldn’t absorb in the stomach and intestine and couldn’t metabolize in the lever and disturbs whole metabolism of the body both elements deposit in the uterus in the fibroids.
The treatment of fibroid is treatable by herbal medicine even third stage of disease treatment should be according to the temperament of the patient and disease.

Line of treatment
1. Mundhij sowda منضج سودا
2. Mushil sowda مسہل سودا
3. Tabreed تبرید
It means first we raise natural heat of the body by changing patient’s diet and life style according to the temperament and prescription of medicine should be according to the patient‘s temperament.
Diet would be 3, 4 number in the diet chart.
Diet should be hot and wet in temperament for diluting sowda or attrabilious .Then excrete the sowda from body by giving laxatives . After that Tabib should suggest cold and wet herbs to neutralize the internal body temperature.
